What to Bring

A valid photo ID (driver’s license or state ID)
Your dental insurance card (if applicable)
A list of current medications
Any recent dental X-rays or records (if transferring from another office)
Your completed new patient forms (available below)
Don’t worry — if you don’t have everything handy, our team will help you complete any missing details during your visit
